Leon Medical Centers has granted you access to the Leon Medi Connect web portal at the request of your employer. You are being provided individual user access pursuant to and in accordance with the Leon Medi Connect Access Agreement between Leon Medical Centers and your employer. By your acceptance hereof, you are agreeing as a user to the terms and conditions of such Access Agreement. If you have any questions concerning the Access Agreement, you should contact your employer. Leon Medical Centers reserves the right, in its sole discretion, to limit or terminate your right to access the web portal at any time for any reason.ö Through this portal you will have access to patient electronic medical records, including Protected Health Information. It is your responsibility to maintain the security and confidentiality of your login credentials once they are provided as you will be accountable for all transactions and accesses performed under this electronic identifier. You will be required to change the initial default password in order to maintain the integrity of the system. After a period of inactivity of 45 days, your account access will be automatically disabled.
